Start With Your Insurance Card
Your insurance card is the best starting point. The back of the card usually lists a member services phone number, and some cards include a separate number for behavioral or mental health benefits, since those benefits are sometimes managed by a different company than your medical coverage. If your insurance comes through an employer, the human resources or benefits office may also be able to explain your plan.
Before you call, gather a few details:
- 01Your member ID and group number
- 02The name and date of birth of the person who holds the policy
- 03The practice name, Infinity Counseling & Wellness Center, and its address: 5241 Fountain Dr. Suite H, Crown Point, IN 46307
- 04Something to write with, so you can record the answers
Questions to Ask Your Insurer
When you reach a representative, ask specifically about outpatient mental health benefits, which some plans call behavioral health benefits. Helpful questions include:
- 01Is outpatient counseling covered under this plan?
- 02Is Infinity Counseling & Wellness Center in network, and is the specific clinician in network?
- 03Is there a deductible, and how much of it has been met so far this year?
- 04What is the copay or coinsurance for an outpatient counseling visit?
- 05Is prior authorization or a referral required before starting?
- 06Is telehealth counseling covered, and do any location rules apply?
- 07Is there a limit on the number of covered sessions each year?
- 08Are family or couples sessions covered the same way as individual sessions?
Common Insurance Terms
A few terms come up again and again when you check benefits. If a representative uses a term you do not recognize, it is fine to ask them to explain it.
- 01In network: the provider has an agreement with your insurance plan to serve its members.
- 02Deductible: the amount you may need to pay for covered services before your plan begins to pay its share.
- 03Copay: a set amount you pay for a covered visit.
- 04Coinsurance: a percentage of the cost of a covered service that you share with your plan.
- 05Prior authorization: approval your plan may require before certain services are covered.
Confirming Coverage With Infinity
Infinity lists accepted plans separately for Indiana and Illinois on the Insurance & Coverage page, with more detail on the Indiana insurance plans and Illinois insurance plans pages. Medicare appears on the Indiana list, and Counseling With Medicare in Indiana covers it in more depth.
Even when your plan appears on a list, benefits can vary from one policy to another. When you call (219) 472-8277 or send a request through the Contact page, the team can help confirm whether your plan is accepted. If you do not see your plan, reach out and ask.
Telehealth and Record Keeping
If you are interested in telehealth, ask your insurer whether it is covered and whether coverage depends on where you are when you connect. At Infinity, telehealth availability depends on the clinician and where you are located during sessions, and the team can explain current options.
Keep a simple record of each call, including the date, the representative’s name and any reference number. Notes like these can be useful if a question comes up later. In the meantime, you can explore Infinity’s counseling services to see which kind of support fits your situation.
